This is the basic information you need to include to dispute any particular publisher's claim.

1) Explain who the rights holder is.
2) Attach contracts, if you have them.
3) Explain that the allocation is incorrect, and this publisher, who is not a rights holder in the work at issue, should get 0%.
This is the document I wrote to explain why they were mistaken. I think pretty much any short summary you provide to them should take this form: explaining who holds rights, explaining what documentation you are providing, and then explaining what you believe the correct allocation should be.
