I think a lot of people assume there are layers and layers of checks at BBC News that simply do not exist anymore. The corp has also massively undervalued the experience that just comes from 30 years of doing a thing. A lot is now 28 year olds in charge of 22 year olds and it shows.
I think this is true and in general the BBC undervalues its backroom staff compared to its on-screen 'talent'. Even before 'basic morality', common sense should have led to Huw Edwards being given less latitude.

But equally being on-screen BBC talent is life-changing and not only in a good way.
I've said it before, but I think parts of the BBC overvalue "talent" (cf, newsreaders) vs the audience pull of the vehicles they are in.
