I think you're overestimating the hospitalization benefits by about 50%. Based on the VA Study (Cai, NEJM 2025), ICER estimated the number at about 750. Not suggesting that would change your conclusions.

Unclear to me why this is causing worries about vutrisiran. In HELIOS-B, the relative benefits with vutrisiran were very similar whether or not patients were on tafamidis. The NEJM article seems to be handwaving that the results are generally consistent with what was seen in HELIOS-B, but

I don't think I know that at all. I'm just not convinced by the report of the radio transmission. The senator's staff could be lying, EMS could have been wrong, or the senator could have made a stunning recovery from a cardiac arrest. That last only seems possible if there were trained staff

That said, "CPR in progress" is unconvincing for cardiac arrest. The majority of the times I've seen someone alive after they received CPR outside the hospital, they had never had a cardiac arrest.

Ignoring any other aspect of this, since when does the USPSTF (that they claim the exam was in accordance with) recommend the various vascular tests that were performed? Carotid US??
