Pretty standard D/S dynamics and appeals different aesthetics.
There is also the obvious piss kink/scat aspect that like. It exists some people are into that.

I don't really get the appeal of the imagery of it in porn.
But for people actually doing it I think it's as simple as performative acts of care being rewarding for the people doing them, and total loss of control including of their basic needs feeling nice for the age regresser.
