75% is a pretty big share, no doubt. But I'd want to know what that actually means for patients. PBMs are doing real work negotiating benefits and managing pharmacy networks, so concentration alone doesn't tell the whole story.

There are definitely things worth fixing, but “nothing will happen” feels like a pretty big conclusion from this. Congress has several different reform options on the table.

Wake up people. Big Pharma has a lot of money behind it, and some of the drug prices we're seeing are hard to justify. Big Pharma should be held accountable about these high prices.

Exactly. You can change how PBMs are compensated, but someone still has to negotiate prices, manage the benefit and keep the pharmacy network running. Those functions don't just vanish.

Exactly. I wouldn't judge a Medicare Advantage plan just by the company name. Look at the actual benefits, drug coverage and out of pocket costs. That's what matters to the person using the plan.

The shift toward clinical services is interesting, but I don't think you can put all of that on PBMs. There’s a lot more going on with drug prices and the cost of keeping pharmacies open.
