I think i had an issue like this on an old laptop and this also happened on the same lag spikes but not as intense as a pause, apparently it was the thermal paste of the GPU running dry and overheating. So if its something like this maybe it just needs replacing?

If you can manage to sneak a look into the task manager while one of those pause looking spikes happen, does the GPU usage also spike to suddendly 99% or 100%?
