In people age 70+ without prior heart attack, bypass surgery, or stenting, the risk of major bleeding events w/ low dose aspirin markedly exceeds benefit. There was an increase in MACE in follow-up of ASPREE. A full review of the data here erictopol.substack.com/p/the-lowdow...

Nuts The ASPREE study, which started as a trial of low dose aspirin in healthy older adults, morphed into a longitudinal study of ageing (Age Ageing doi: 10.1093/ageing/ afae239). It recently reported that people who eat nuts every day tend to have a longer disability-free survival. The explanation may be nutritional, because nuts are rich in vitamins, minerals, polyphenols, phytosterols, and unsaturated fats. Another possibility is that the sort of people who choose to eat nuts are the sort of people who have a generally healthy way of life.

A tweet by Eric Topol discusses the long-term effects of daily low-dose aspirin for older adults without prior cardiovascular disease (CVD) events, based on the ASPREE trial and its extended follow-up. The findings suggest the recommendation for aspirin in this demographic is ill-founded. Older adults randomized to daily low-dose aspirin in the ASPREE trial had a higher rate of major cardiovascular events (MACE) during the post-trial period compared to those on a placebo. Over the entire study period, a higher rate of major hemorrhage (bleeding) was observed in the aspirin group. Clinical decision-making for prescribing aspirin for primary prevention in older individuals should consider established bleeding risks and the potential lack of long-term cardiovascular benefit.
