Wow. Increased major cardiovascular event with abacavir compared to TDF and TAF up to 40-50%. Absolute numbers low but still.
Does this make ABC obsolete? Do we switch everyone off based on this if they have alternative options?

Introduce yourself as what almost killed you

Hi, I’m the rogue waves that dragged me into the Mid Atlantic, I’m the bacterial endocarditis, sepsis & kidney failure that arrived after a Baltic cruise, I’m the severe allergic reaction to Abacavir that crashed my blood pressure. Pleased to meet you!
Introduce yourself as what almost killed you:

Hello, I’m the unseen tram, the drunk driver, the concrete on the other side of the wall, the arrow, the electric shock, the severe illness, the black ice, the psychotic ex, and middle aged ennui.
Introduce yourself as what almost killed you:

Hello, I’m streptococcus bacteria sitting on your previously undiagnosed bicuspid aortic heart valves. Planning on doing any breathing.

And I’m still the reason you’re sitting in a hospital right now.

Does ART matter with relation to CVD? Yes.

Abacavir is bad for people experiencing or at risk for CVD.

So EACS has followed DHHS and relegated abacavir to 'alternative' 1st line status. Time for @bhiva.bsky.social to do the same? If you look at consultation comments @tristanjbarber.bsky.social called for this last time round.
BHIVa had already relegated raltegravir mind.

I’m afraid they have an M184V mutation that would make the Abacavir and Lamivudine ineffective (VF=virologic failure).

In general I try to avoid Abacavir because of the cardiovascular effects. This pt was on it when they came to me.
