State prioritizes opioid response through collaborative interventions and community funding for prevention efforts

Legislative leaders said the $272 million federal Rural Health Transformation award could help reshape care in Alaska but raised concerns that most program policy changes were committed by the Department of Health without prior legislative consultation and that key rules and CMS scoring remain opaque.

University of Alaska Anchorage officials told the House Health and Social Services Committee they are seeking $2 million in year-one funding as part of a $30 million rural health transformation effort to study and plan an Alaska academic medical center, expand residencies and retain more graduates in-state.

The Senate Budget Subcommittee on April 17 adopted the Department of Health VA (budget action) report for FY27, approving a $377,298,200 increase to Medicaid, new IT funding, and targeted additions for food banks and the Infant Learning Program while declining an $8 million virtual call center request.

House Finance subcommittee staff told lawmakers the Department of Health budget would increase about $460 million (12.1% over FY26) largely because of Medicaid federal receipt authority, and highlighted investments in eligibility systems and a $625,000 increment for private duty nursing intended to reduce Medicaid inpatient costs.

The subcommittee moved the Department of Health FY27 operating BA report out of committee after adopting targeted time-limited federal-receipt authorities and debating proposed position deletions and program cuts; members also approved restores for behavioral health grants, a GF match for the crisis call center, and one-time food-bank funding.

The Department of Health presented a $4.52 billion FY27 governor's amended budget, highlighted Medicaid and eligibility-system investments, and defended responses to 18 FY24 audit findings while lawmakers pressed on sustainability of rural transformation funding.

House Bill 285 would update Alaska's Health Information Exchange law to clarify department duties, data uses, patient authorization options and add behavioral health representation to governance. Testimony focused on patient control, EMR connection fees, de-identification limits and system reliability.

Deputy Commissioner Emily Ricci told the subcommittee that H.R. 1 now requires an actuary review for Section 1115 waivers and that Alaska’s current demonstration meets existing budget‑neutrality practice; the department described Guidehouse/Milliman rate work and next steps for public comment and implementation.

Department of Health officials told the House Finance Committee the governor’s amended FY27 budget increases the department’s operating total to $4.3 billion and requested supplemental Medicaid authority (including $334 million federal and $42,288,100 general fund). Officials also summarized HR1 changes to SNAP and Medicaid, estimating about 69% of Medicaid expansion enrollees would qualify for automatic exemptions from new work/engagement requirements.

The Department of Health briefed the Senate Health and Social Services Committee on the federally funded Rural Health Transformation Program, saying Alaska received $272 million in year-one funding and describing program goals, allowable uses, reporting, oversight by CMS, and application pathways.
