Another option is that some tiny piece of dirt or some other "inclusion" found its way into the rail-weld interface. Or that the cut made prior to the weld wasn't made cleanly.

Again, not in and of itself an issue.

However, for an aluminothermic weld like the one that failed, you need to use a harder R350HT kit to ensure the right hardness at the running surface over time.

My Spanish isn't good enough to accurately translate the Adif standards...

Aluminothermic welds as developed by Hans Goldschmidt in the 1890s use what's called a "portion" to link rail ends thanks to a self-sustaining chemical reaction between aluminium powder and a metal oxide of some kind (this mix is called "thermite").

Not a stupid question at all. A likely culprit given the position of the break is that the heating associated with aluminothermic welding was not properly managed creating a weaker crystalline structure in the steel adjacent to the weld (in an area called the heat affected area).
