358 Associations of area deprivation and social vulnerability indices with pediatric vision screening outcomes in the UCI EyeMobile for Children Program
Area Deprivation Index (ADI) and Social Vulnerability Index (SVI) are nationally validated neighborhood measures associated with pediatric health outcomes. Evidence in pediatric ophthalmology is limited. To our knowledge, this is the first study to examine ADI/SVI with school-based vision screening outcomes, refractive error (RE), amblyopia, and best-corrected visual acuity (BCVA).

EyePoint Announces Topline Data from LUGANO, the First of Two Pivotal Phase 3 Clinical Trials for DURAVYU™ 2.7mg in Wet AMD
EyePoint (Nasdaq:EYPT) reported topline results from LUGANO, the first pivotal Phase 3 trial of DURAVYU™ 2.7mg in wet AMD versus on‑label 2 mg aflibercept. The primary endpoint (BCVA change) was not achieved in the full dataset, affected by a 4% asymmetric cohort (9/211 DURAVYU patients) with ≥15‑letter vision loss unrelated to wet AMD; no such cases occurred in the aflibercept arm. An ad hoc analysis excluding this cohort showed non‑inferiority for DURAVYU (nominal p=0.0096).Key secondary endpoints favored DURAVYU, including a 42% reduction in treatment burden (two fewer injections on average; p<0.0001), high supplement‑free rates (76% to Week 32; 54% to Week 56; 79% with 0–1 supplement to Week 56), non‑inferior BCVA in supplement‑free patients (p=0.0035), strong anatomic control with a 4‑micron CST difference at Week 56 (3 microns in supplement‑free eyes), and a favorable safety profile with repeat dosing. EyePoint expects topline data from the second pivotal wet AMD trial, LUCIA, in Q4 2026 and is planning a potential FDA NDA submission for DURAVYU in 1H 2027, pending those results. Phase 3 DME trials COMO and CAPRI have fully enrolled, with topline data anticipated in Q4 2027.

Novel classification for microspherophakia and its related surgical implications: a retrospective cohort study
Background A novel classification for microspherophakia (MSP) was developed using a nomogram model to stratify patients and evaluate surgical outcomes. Methods The study involved 99 eyes from 64 MSP patients, divided into training (64 eyes) and validation (35 eyes) cohorts. Primary outcome included the best corrected distance visual acuity (BCVA) at the 1-year follow-up, and the secondary outcome was intraocular pressure (IOP) control. MSP patients achieving a BCVA of ≤0.2 logarithm of the minimal angle of resolution with successful IOP control were categorised into the favourable outcomes group, while those not meeting these criteria were placed in the unfavourable outcomes group. A nomogram model was constructed to predict unfavourable outcomes by identifying associated risk factors. Results In the training cohort, 39.06% (25/64) achieved favourable outcomes, while 60.94% (39/64) had unfavourable outcomes. Decreased anterior chamber depth (ACD) and the presence of glaucoma were identified as risk factors for predicting unfavourable outcomes. The concordance index for the training and validation cohorts was 0.828 and 0.768, respectively. A nomogram-derived cut-off value of 56 points stratified patients into low-risk (≤56 points) and high-risk (>56 points) groups. Postoperative BCVA was significantly better in the low-risk group at 6-month and 1-year follow-ups. Conclusion MSP patients in the low-risk group achieved satisfactory outcomes. For high-risk patients with shallowing ACD, early surgical intervention is recommended to prevent secondary glaucoma. Close monitoring of IOP and glaucoma progression is crucial for patients with secondary glaucoma. This classification offers valuable insights for outcome stratification and guidance in clinical decision-making for MSP management. However, the limitations of this study include its small sample size, which may impact generalisability, and future studies with larger cohorts are needed to validate and refine the nomogram.

Natural history of progressive vision loss in Bietti crystalline dystrophy: a model-based meta-analysis
Purpose Bietti crystalline corneoretinal dystrophy (BCD) is an autosomal recessive progressive retinal degenerative disease due to mutations in the CYP4V2 gene. Best-corrected visual acuity (BCVA) is a common primary endpoint in clinical trials for retinal diseases, but the natural history of BCVA loss remains unclear because of the heterogeneity of manifestations in BCD patients. Methods Based on the individual data of untreated BCD patients, a disease progression model was established using the change in BCVA from baseline as an index, and covariates including age of onset, age, duration of disease, baseline BCVA, gender, race (East Asian/non-East Asian), genotype, and family history. Then, based on the final model, the natural disease progression characteristics of BCD were simulated. Result A total of 14 studies met the inclusion criteria, with a total sample size of 117 cases, including 6 studies (N=80) with East Asian populations and 9 studies (N=37) with non-East Asian populations. The change of BCVA from baseline increased linearly with time, and the disease progression model of BCD was successfully established. BCVA increased by 0.06 logarithm of the minimum angle of resolution (LogMAR) per year in BCD patients. BCVA increased by 0.09 LogMAR per year in patients with BCVA≥0.5LogMAR and disease duration more than 10 years. Conclusions For the first time, we successfully established a BCD disease progression model based on the change in BCVA from baseline. The mean visual acuity loss increased linearly with the progression of the disease. A sharper loss of BCVA may be expected in patients with BCVA≥0.5LogMAR and disease duration ≥10 years.

Transcranial magnetic stimulation as a novel therapeutic approach for severe retinal degenerative diseases: a pilot study
Background Retinal degenerative diseases, including myopic macular degeneration (MMD) and retinitis pigmentosa (RP), lead to irreversible vision impairment, with limited treatment options. Transcranial magnetic stimulation (TMS), a non-invasive brain stimulation technique for rehabilitation, may hold potential for vision restoration. The pilot study explored whether TMS is associated with short-term functional visual improvements in patients with severe retinal degenerative diseases. Methods This prospective, non-randomised, parallel-arm pilot study enrolled 98 patients with bilateral severe MMD or RP. Participants received either five consecutive days of theta-burst TMS or conventional conservative care. Primary outcomes were changes in best-corrected visual acuity (BCVA) and visual field from baseline to 4-week follow-up. Contrast sensitivity, macular sensitivity, fixation stability, Visual Function Index-14 (VF-14) and Visual Functioning Questionnaire-25 (VFQ-25) questionnaires were also evaluated. A subset of participants underwent resting-state functional magnetic resonance imaging (fMRI) to explore cortical functional changes. Results The study analysed 174 eyes from 88 patients (mean age: 60.3±9.1 years; 52% female). Compared with controls, the TMS group showed greater improvements in BCVA (MMD: 0.22±0.28 logarithm of the minimum angle of resolution (logMAR); RP: 0.24±0.21 logMAR) and VF mean defect (MMD: 1.96±2.09 dB; RP: 1.08±1.28 dB). Contrast sensitivity, macular sensitivity and fixation stability also showed short-term gains. Patient-reported outcomes improved in several functional domains. fMRI demonstrated increased activity and functional connectivity in visual-related cortical regions following TMS.Conclusions In this exploratory pilot study, TMS treatment was associated with short-term functional visual improvements and cortical modulation in individuals with severe retinal degenerative diseases. These findings are hypothesis-generating and warrant validation through larger, randomised, masked clinical trials with longer follow-up.
