We also offer a one day rapid low dose start based on a protocol by Dr. Jan Widerman - wait 6 hrs since last opioid use, dose every 2 hrs : 0.5mg x 4, 1mg x 2, 2mg x 2, then 8mg

(We can’t easily get Brixadi for insurance & practical reasons)

There are newer ways to administer the medication: Sublucade and Brixadi. Once a month or once a week (usually) injection that can be done at a clinic. No need for daily dosing at sober living and teeth worries.

We need to pivot from methadone to long-acting suboxone injections (e.g. Sublocade, Brixadi).

Sublocade appears much more effective and much less logistically burdensome than methadone.

It is unconscionable that people suffer from opiate withdrawals within hospital walls. There are a couple of fantastic and immediate treatments for this: Suboxone and methadone, then followed by Sublocade and Brixadi. Within 6 months they can be free from addiction. Do you see this advertised?! No!

MAT. Many of our patients are on oral suboxone (the rest get Vivitrol, Brixadi, or Sublocade injections) and lately we’ve been getting so many rejections from insurance companies for the stupidest non-reasons.

Nobody tells you this before you start: the shot for opioid addiction comes in 3 totally different versions.

Vivitrol forces full detox first. Sublocade doesn't. Brixadi offers weekly options.

We have a similar microdosing protocol that I could probably send. I don't think we have access to Brixadi but we have had great success with our microdosing initiations (besides primary teams being frustrated how long they stay in the hospital to get to their bupe dose).
