AbstractIn an international cohort of hospitalized patients included in the Multi-Drug-Resistant Organism Network with carbapenem-resistant Gram-negative infections, increased age-adjusted Charlson Comorbidity Index scores were associated with increased mortality particularly in those with respiratory and less so in those with bloodstream infections. Cirrhosis, diabetes, and peripheral vascular disease were important contributors.

Background: The Charlson Comorbidity Index (CCI) is widely used to quantify comorbidity burden in observational #research. Applying it to real-world data requires accurate International Classification of Diseases–based mappings. The commonly used mapping does not reflect current German coding standards. Objective: This study aimed to develop year-specific mappings (2004 to 2026) based on the German modification of the International Classification of Diseases, 10th Revision (ICD-10-GM), for the CCI with an open-source implementation and compare them with the commonly used mapping. Methods: For each year, mappings were curated via independent dual review with consensus. In 515,827 in#patient cases from a German tertiary center (2010-2024), year-appropriate mappings and the commonly used mapping were applied, and the results were pooled into 2 corresponding datasets that were subsequently compared. Agreement was assessed using the intraclass correlation coefficient (2-way mixed-effects model for absolute agreement based on single measurement), and the significance of the discordances was measured using the McNemar test. Results: Agreement was very high; 3.1% (16,136/515,827) of cases differed. A single code was present in 48.9% (7896/16,136) of the discordant cases and was intentionally excluded in the new mappings as it was not diagnostic for the category it was assigned to. Discordant results were found in 58.8% (10/17) of the categories (“peripheral vascular disease,” “mild liver disease,” and “severe liver disease”) being present in, respectively, 51.7% (8339/16,136), 17.4% (2813/16,136), and 16.4% (2657/16,136) of the discordant cases. After filtering the dataset to only include observations with any diagnosis of a malignant disease, 6.9% (9377/136,607) of cases differed, and 79.3% (7434/9377) of those differences were caused by the “peripheral vascular disease” category, suggesting significant discrepancies for specific #research questions. Conclusions: We provide transparent, year-specific ICD-10-GM (2004-2026) mappings and an open-source tool for CCI calculation. The mappings align with current German coding practice and enable reproducible, year-appropriate comorbidity adjustment using administrative data.

AbstractObjectivesTo identify predictors of all-cause 30-day mortality in bloodstream infections (BSI) caused by carbapenem-susceptible Escherichia coli and Klebsiella pneumoniae, with emphasis on bacterial species and the extended-spectrum β-lactamase (ESBL) phenotype.MethodsThis was a single-centre retrospective cohort study of adults with a first episode of BSI due to carbapenem-susceptible E. coli or K. pneumoniae at a referral hospital in Lima, Peru (June 2020–December 2023). Primary outcome: All-cause 30-day mortality. A multivariable Cox model was built, with covariate selection guided by a directed acyclic graph, and the species × ESBL interaction was tested.ResultsOf the 403 patients (median age 66 years, 55.8% men; 58.6% ESBL+), 83 (20.6%) died. Mortality was higher in patients with K. pneumoniae than in those with E. coli (26.8% versus 17.4%; log-rank P = 0.018). In the multivariable analysis, K. pneumoniae was an independent predictor [adjusted HR (aHR) 1.72; 95% CI, 1.09–2.73; P = 0.021], as was a Charlson score ≥6 (aHR 8.43; 95% CI, 3.33–21.3; P < 0.001). ESBL was not an independent predictor (aHR 1.45; 95% CI, 0.92–2.29; P = 0.113), nor was the species × ESBL interaction (P = 0.43). The effect of K. pneumoniae was consistent across all sensitivity analyses (aHR 1.66–1.94).ConclusionsK. pneumoniae nearly doubled mortality compared with E. coli, independently of ESBL, which did not predict mortality in this cohort of carbapenem-susceptible isolates. These findings challenge the assumption of prognostic homogeneity within carbapenem-susceptible Enterobacterales and support species stratification.

Campylobacter spp bloodstream infections (BSIs) are an emerging clinical challenge, yet information on their clinical features remains limited. We aimed to evaluate their differential clinical characteristics and outcomes in immunocompetent (IC) and immunodeficient (ID) patients.MethodsA multicenter, multinational, retrospective cohort study of consecutive Campylobacter spp BSIs between 2009 and 2024 was carried out. Demographics, Charlson comorbidity index, types of immunodeficiencies, clinical characteristics, and antimicrobial susceptibility were analyzed. The primary outcomes were relapse or reinfection and death (all-cause mortality outcome).ResultsA total of 261 episodes were included, 89 (34.1%) in IC and 172 (65.9%) in ID patients. Among the 172 ID patients, 104 (60.5%) and 68 (39.5%) had humoral or nonhumoral immunodeficiency (primary or secondary), and 42 (24.4%) had primary immunodeficiencies (PIDs). Patients with humoral immunodeficiency (humoral-ID patients) were younger (median age, 40 years [interquartile range, 23–66 years]), had lower Charlson comorbidity indexes (2 [1–5]), and had a higher frequency of previous Campylobacter spp infection. Resistance rates to fluoroquinolones (75%) and macrolides (14.5%) were high. BSI relapse or reinfection occurred in 21 (8.0%) patients, mostly humoral-ID patients (n = 19 [18.3%]), 11 (57.9%) of whom had PID. Death occurred with 30 days (all-cause mortality outcome) in 26 patients (9.9%) across the entire cohort; the 30-day mortality rate was lower in humoral-ID (n = 5 [4.8%]) than in IC (n = 12 [13.5%]; P = .05) patients.ConclusionsHumoral-ID patients and those with Campylobacter spp BSI were younger and had fewer comorbid conditions, and Campylobacter coli was the most frequent species. Relapse or reinfection was common in humoral-ID patients, particularly those with PID. The antimicrobial resistance in C coli and Campylobacter jejuni represents a clinical concern.
