"All according to keikaku" was actually a meme mocking bad anime fansubs of the time and their overreliance on translator notes. It's not something a Death Note fansub of the episode did for real, they all translated "keikaku doori" with some variation of "Just as planned."
What's your "He actually broke his toe here" thing? The factoid you can't help but share whenever it comes up.
For me, it's how the punk on the bus in Star Trek IV actually wrote and recorded the song that he's playing because he was annoyed the original song they had didn't sound punk at all.

I keep finding out about Retro Games exclusive to Japan that I really, REALLY want to play.
The 1999 Playstation game, 'Yuuyami Doori Tankentai' or 'Twilight Road Investigation Party' is one of them. It looks SO interesting!
It also has some of the most unsettling cover art I've ever seen.
