TFW the ER doctor knows what MCAS is and jumps right to giving you a liter of fluid plus benadryl and famotidine 😭🥹😭
(I declined the benadryl to start since iv famotidine is usually super effective for me, since my MCAS is so gut-centric, but relieved to know she's game to add it)

I have MCAS and take loratidine, cetirizine, and famotidine twice daily. For COVID prevention, loratidine or cetirizine + famotidine is probably good enough.

-20 mg cetirizine or loratidine
-20 mg famotidine
-Every 12 hours
-Until you are completely recovered

Adding famotidine (a H2 antihistamine) also can help; max dose there is 40mg/day. I'm creeping up on the line between mild and moderate MCAS and my current baseline is 10mg cetirizine + 10mg loratidine + 20mg famotidine every 12 hours + like 5 other things as MC stabilizers, sigh.

Many countries don't have famotidine or cimetidine available OTC and it needs a prescription. Note: proton pump inhibitors (another type of antacid) are NOT H2 antihistamines and won't work for COVID prevention.

Pepcid = famotidine, Zantac = the now withdrawn ranitidine (RIP), Zantac 360 = famotidine, Reactine = cetirizine, if that helps any!

I've suspected mast cell involvement in COVID ever since I suddenly couldn't get famotidine for six months in 2020 because Sinai did their medication review of patients in that first wave and discovered famotidine + H1 antihistamine strongly correlated with improved outcomes.
