for IgG mediated diseases, this means that FcRn also recycles pathogenic IgG autoantibodies, which perpetuates disease. so targeting FcRn can reduce circulation of IgG autoantibodies and reduce disease symptoms.

the neonatal Fc receptor (FcRn) plays a key role in recycling IgG antibodies. basically, FcRn binds to IgG so that they don’t go to the lysosome where they are destroyed. this allows bound IgG to re-enter circulation.

FcRn protects IgG and albumin from catabolism, transports IgG across the placenta, and regulates IgG function. Blocking or augmenting FcRn can decrease autoimmune antibodies and enhance drug delivery. Learn more in a new Review Article: nej.md/42gEZYk

This is some of the most exciting observational research into post-viral illness. At the end, @virusesimmunity.bsky.social says that positive results would lead them to try to pursue trials with immunotherapies like FcRn inhibitors, B cell targeting therapies, and IVIg.

The neonatal FcRn receptor, which persists in adult life, can be exploited both ways. In a Br J Haematology paper, its inhibition is exploited as a treatment of #ITP. In congenital #hemophilia, it is used to enhance the half-life of FVIII and FIX. tinyurl.com/c46hm9nv

2) Rozanolixizumab is a monoclonal antibody that targets the neonatal Fc receptor (FcRn). This receptor normally protects antibodies from breakdown, so blocking it leads to a reduction in antibodies.

The drug is already approved as a treatment for myasthenia gravis.

US20260274961A1 discloses dual-variable-domain IgGs targeting FcRn and activating FcγR to redirect immune-complex handling. FcRn/FCGR2A knockout and transgenic models, including K/B×N arthritis, test receptor dependence. Key risk: immune-complex clearance may amplify myeloid inflammation.
