Yesterday I took my (I hope) final fingolimod pill for MS. My neurologist says he’s had great success weaning people (with five years of no relapses or new MRI activity) off of their MS medications once they turn 55.

Both fingolimod and ocrelizumab are FDA approved for treatment in adult multiple sclerosis. Only fingolimod is approved for treatment in POMS. This retrospective study explores the safety and efficacy of fingolimod & ocrelizumab in treatment of POMS

Should dosing of gilenya be weight or monitoring based? Despite a set dose of 0.5 mg once daily, levels of the active metabolite fingolimod phosphate ranged nearly 10 fold from 0.48 to 4.28 µg/L in this study, and it correlated with lymphocyte count. www.msard-journal.com/article/S221...

The disease course can vary among patients, with the most common form being relapsing-remitting MS (RRMS), which is characterised by clearly defined relapses of new or worsening neurological symptoms. MS is one of the most common causes of neurological disability in young adults aged between 20 and 40 years, with approximately two-thirds of cases occurring in women (Hauser and Cree, 2020; Dobson and Giovannoni, 2019; Oh et al., 2018; Karussis, 2014).

MS rebound disease can occur very quickly after stopping gilenya. Here is a case report of a 15 year old boy who developed a severe flare affecting cognition, balance, and vision 4 weeks after stopping gilenya (who had planned to switch to ocrevus) www.thieme-connect.de/products/ejo...

🧪 #Amiselimod demonstrated a more favourable cardiac safety profile than fingolimod over 28-day administration.
🧪Unlike fingolimod, there were no clinically relevant cardiac adverse effects observed with amiselimod at doses up to 0.8 mg. Dose titration was not necessary.
