The goal of the TGP Study is to create a large-scale de-identified database and biological sample bank that can be used for future research studies of human health and disease and finding better ways to keep people healthy and to help them when they are sick. Being a part of the TGP Study is your choice. If you choose to be part of this study, you agree to: The collection and use of a “Biological Sample” for research. This will be a leftover sample from a clinical procedure ordered by your healthcare provider that is no longer needed for your care. A blood sample will be drawn for tests ordered by your doctor. After the tests have been done, the leftover blood will be used for the study sample. Studying your Biological Sample in a lab using scientific tools and methods, like whole-genome sequencing or other ways to study the proteins and chemicals in it. The information collected from studying your Biological Sample is called “Sample Data”. Allow Trinity Health to share identifying health information from your medical record (“Health Information”) with Truveta. Storing your Biological Sample for an unlimited period of time in a TGP Study biobank. Sharing your de-identified Health Information, Sample Data, and/or Biological Sample with Research Partners to use in future research on a wide variety of research topics, including genomic analyses. “Research Partners” are researchers at Truveta, Regeneron Genetics Center and its affiliates (“Regeneron”) Screenshot of portion of agreement: IS MY HEALTH INFORMATION PROTECTED AFTER IT HAS BEEN GIVEN TO OTHERS? With due respect, I humbly suggest terms excluding the #prejudicial & broad–brush painting descriptors,‘white‘, ‘black‘, and ‘yellow‘; I recommend simply (Euro-) #supremacist; I’ve been advocating—since the #GenomeProject —ending use of #VictorianScience terms. #HuesSubjugate; they don’t liberate.
