We had our home room teachers sign them every day. I went to self-paced HS so planning was everything. The whole point of homeroom was chat with fellow students and that teacher about time management. Paper is good, actually?

Personal Info - I had a hard time with this one. Gamey dungeon crawlery players kind of ignore this. So I decided on a quick single roll (Blood Type) to determine personality for any NPCs, PCs, etc.
Guardian and Homeroom teacher can use the Blood Type roll and also hook into quests.
