Ibrahim Abdullah, 5, suffers from the rare genetic disorder Primary Hyperoxaluria Type 1, which has destroyed both of his kidneys.
His seven-year-old sister, Naima, is also affected by the same genetic disease.
The two siblings urgently need a medical transfer out of Gaza.

Interesting new publication from the main journal looking at the use of siRNA based therapies in Hyperoxaluria, discussing why some may not work as well as we hoped they would.

YOLT-203 is an in vivo gene-editing therapy targeting primary hyperoxaluria type 1, a rare inherited metabolic disorder.
YOLT-204, is designed to target the bone marrow with a simple IV infusion, which could provide a safer and more scalable treatment for blood disorders.

So needless to say, I am going to drown myself in homemade kefir on the off chance I've grown the strain at home, and get back to my artsy to-do list so I can make that medicine happen!

I just threw all this info out there in case it helps the other rare hyperoxaluria patients out there. ☀️✨

Having a rare disease (hyperoxaluria) means sometimes you figure things out about it as you go.

I learned the hard way antibiotics kill off a specific good bacteria needed for this condition (oxalobacter formigenes) which is why I've been outrageously sick lately! 😷

I've always assumed that people who drink ethylene glycol do it because they've internally asked themselves, "I wonder what it's like to have primary hyperoxaluria, types 1, 2, or 3?"
Ethylene glycol intoxication presents w/ anion gap metabolic acidosis & acute oxalate tubulopathy. Detection of urinary calcium oxalate monohydrate crystals helps in diagnosis [not classic dumbbell nor envelope shaped (dihydrate) but rather “cigar” shaped]. Slide courtesy of @jrseltzer.bsky.social

Fun fact: cystinuria was the first of the genetic kidney stone diseases to be characterized. Garrod presented his findings at the 1908 Croonian Lectures in London. Primary hyperoxaluria was the next to be described, and that was in 1925 by Lepoutre.
Cystine crystals (bright field, x400).

Their presence in #urinarysediment is pathognomonic for #cystinuria, the most common kidney stone disease with Mendelian genetics.

While perfectly specific, the sensitivity of cystine crystalluria is quite low.

The fact that primary hyperoxaluria is the first kidney disease for which an RNA therapeutic has become available is a milestone for the field.

How does this change the treatment landscape? 🧵

Oh god jesus fuck do people have no conception of runaway redox catalysis? Also fucking cofactor depletion. Ahhhhhh. Dumbasses need more metabolic dieticians on board.

And yeah I have primary hyperoxaluria and it is worse than kidney ... excess C fucks up liver cycling of AGXT related proteins.
