Want to increase efficiency? Take an international, collaborative approach to vetting evidence and developing guidelines so that we don't keep reinventing the wheel in every setting. —@Tammy_Hoffmann #KTCan18

Where possible, display benefit–harm tradeoffs using patient-important events. Present it in a way that helps clinicians have those conversations with patients. —@Tammy_Hoffmann #KTCan18

How does patient and public involvement influence guideline development? Qualitative study found PPI always brings back the issue of impact, all the way to how guidelines are presented. —@Tammy_Hoffmann #KTCan18

Even after an intervention is recommended, clinicians may not implement it because they feel they don't have the skills. —@Tammy_Hoffmann #KTCan18

I was going to reissue this tweet for my new followers from #KTCan18, but given my #OntarioVotes reactions today, I'm thinking a warning about my potty mouth is no longer necessary.
Welcome, new followers from Fuse! 👋 I should warn you that although I do tweet about health, I also tweet about a lot of other random bullshit—like Bigfoot, cake, and ceremonial ribbon-cutting scissors—and I swear like a motherfucker. (Run while you still can.)

What makes a useful systematic review? Again, we need to know: (a) what was the intervention and (b) what were the benefits and harms? If information is missing from a study, it will also be missing from the SR, but in the SR, the problem is magnified. #KTCan18

So if these elements aren't all described in the reporting, it can be hard for informed decision making. —@Tammy_Hoffmann #KTCan18

Harms are often framed as adverse effects, but from a patient perspective, harms should be considered more broadly: cost, time commitment, burden or travel, interruption to work and daily lifestyle. (YES!) —@Tammy_Hoffmann #KTCan18

Trials need to report benefits AND harms. Harms are traditionally undermeasured, underpowered, and underreported. —@Tammy_Hoffmann #KTCan18

We assume the limits of a patient's contributions to a research team—without asking the patient or presenting a menu of options. Need to ask what's the best fit for where their passions lie. #KTCan18

Shared decision making is actually a component of evidence translation. Applying the evidence requires discussion with patient for optimal patient care. Need skills in evidence but also skills in patient communication. —@Tammy_Hoffmann #KTCan18

For example, engagement is probably much more important for an intervention project compared with a methods project. #KTCan18

Does an intervention work? Need to help patients weigh benefits and harms. Which of several options is "better"? Depends on the patient's preferences. —@Tammy_Hoffmann #KTCan18

.@BCSUPPORTUnit aims to help teams to do patient-oriented research well. It would be nice to give a recipe—"you need to do this in this circumstance"—but we can't, because it depends on your situation, your population. #KTCan18

Compared with usual care, patient decision aids lead to 13% higher knowledge, 110% more accurate risk perception, 106% better match between values and choices & they reduce decisional conflict. #KTCan18

Language with patient engagement is very important. @CherylKoehn asks researchers to choose their words carefully—and to make it clear to people that patient engagement is not going away. #KTCan18

Dawn Stacey now talking about patient engagement at the direct care level. Shared decision making:
•patients need to know there's a decision to be made
•need information exchange
•need to know patients' values and preferences
