Young-onset colorectal tumors have different cellular neighborhoods. Researchers mapped nuclei from 189 tumors across ages 29-90 and found younger patients' microsatellite-stable tumors contained more stromal cells and fewer epithelial cells. Pattern confirmed in 712 additional tumors.

Insights into early-onset colorectal cancer from a single-nucleus RNA-seq atlas of colorectal cancer across the age continuum
We profiled 54 EO-CRC and 135 AO-CRC primary tumor samples from banked frozen resection specimens using 3′ snRNA-seq (Fig. 1a), with >20 samples in most age decades (Fig. 1b). Tumors were annotated for patient sex, primary tumor sidedness, microsatellite status, neoadjuvant treatment history, stage and Lynch syndrome status (Fig. 1c and Supplementary Table 1). Consistent with prior reports9,12, tumor sidedness and stage were significantly associated with age in our cohort (chi-square test, two-sided; sidedness: χ2 = 19.3, degrees of freedom = 2, false discovery rate (FDR) = 0.00032; stage: χ2 = 13.0, degrees of freedom = 3, FDR = 0.016), with more left-sided tumors (Fisher’s exact test, two-sided; odds ratio (OR) 3.61, 95% confidence interval (CI) 1.85–7.04, P = 0.0002) and stage IV tumors (OR 2.55, 95% CI 1.23–5.27, P = 0.017) in EO-CRC compared to AO-CRC. There was no significant association between age and sex, microsatellite status or neoadjuvant treatment history in our cohort. From the 189 patient tumor samples, we assembled an snRNA-seq CRC atlas of 850,222 high-quality nuclei (median per sample: 1,582 genes, 2,408 unique molecular identifiers (UMIs), 4,483 nuclei, 0.25% mitochondrial UMIs; Extended Data Fig. 1a and Supplementary Table 1). Adagene Announces Dosing of First Patient in Phase 1 Study of Muzastotug in Combination with Incyte’s INCA33890 in Patients with Microsatellite Stable Colorectal Cancer
Adagene (ADAG) announced first-patient dosing in a Phase 1 study combining muzastotug with Incyte’s INCA33890 for microsatellite stable colorectal cancer. The open-label trial, in which treatment is known, includes dose escalation and expansion in third-line patients whose cancer progressed on prior therapies, with or without liver metastases, meaning cancer spread to the liver. Incyte is sponsoring and conducting the study, while Adagene supplies muzastotug under the collaboration announced in April 2026. Adagene said the study will evaluate whether the combination can deepen and sustain antitumor responses with an acceptable safety profile.

hypothesis= genome size-changing mutations occupy a very diverse range of s; many have a very small s, well below 1/Ne, even for the highest Ne's (eg, microsatellite expansion); some have a very large s, well above 1/Ne, even for the lowest Ne's (eg segmental duplications) (2/3)
