NICAM investigated the drug nilotinib for people with advanced melanoma. Nilotinib may be helpful for people with a specific genetic alteration in their cancer, which could be found with blood tests instead of using samples taken directly from the cancer.

Dasatinib and nilotinib are front line Tyrosine Kinase Inhibitors (TKIs) used to treat BCR-ABL+ B cell acute lymphoblastic leukemia (B-ALL) and BCR-ABL+ chronic myelogenous leukemia. We previously showed that combining nilotinib with anti-PD-L1 blockade significantly reduced leukemia relapse. The TKI dasatinib is more commonly used to treat to B-ALL. However, unlike nilotinib, dasatinib also inhibits SRC-family kinases, which may make it less efficacious in combination with anti-PDL1 blockade. Herein we assess the impact of nilotinib versus dasatinib on anti-leukemia immune responses. Dasatinib, but not nilotinib, inhibited T cell proliferation at high doses in-vitro, but neither TKI significantly impacted T cell function or expansion in response to immunization in-vivo with a model antigen (2W1S) plus polyIC-adjuvant. Dasatinib and nilotinib both reduced leukemic blasts equivalently after 5 days of treatment. In contrast, nilotinib and PD-L1 blockade, but not dasatinib plus anti-PDL1,

When I was a BMT nurse in the 90s the most common indication for transplant was CML & Md survival was 2-7 years.
Gleevec changed that.
Now most pts take oral tx & have near normal lifespan.
I can’t tell you the joy of knowing that pts have hope like this.

11 CONCLUSIONS AND FUTURE DIRECTIONS In 2024, CML experts and patients with CML have multiple treatment options in the CML therapeutic armamentarium, including six TKIs (imatinib, dasatinib, bosutinib, nilotinib, ponatinib, and asciminib), omacetaxine (protein synthesis inhibitor), and several older agents (hydroxyurea, IFN-a, cytarabine, hypomethylating agents, and others). Most patients with CML would be expected to have a near-normal life span and be potentially functionally as well as molecularly cured with optimal TKI therapy. Future directions will focus on strategies to increase the potential of molecular cure of CML (i.e., achievement of a durable DMR and TFR).174 This is not a trivial issue since, with effective TKI therapy and full TKI treatment penetration worldwide, the prevalence of CML would increase annually and plateau around 2040 at a rate of 35 times the incidence. This may represent a considerable challenge in relation to drug availability/affordability, compliance, long-term side effects, and costs. The TFR rates may increase with the current more potent new generation TKIs alone, or in combination with other available targeted therapies. Further understanding of the pathophysiologic events downstream of BCR::ABL1 may help in the development of new strategies to target them.
