I have no idea if it would be any use at all on delayed sleep phase disorder, or on non24 that's due to light perception, or shorter-phase non24, but there are trials running for use in various sleep disorders (check clinicaltrials.gov for "solriamfetol" and "recruiting" if curious).

You will need your doctor to also code your non24 as "excessive daytime sleepiness" for your insurance to be likely to cover it, and it *is* expensive if it doesn't (about $1k/month). But it's not like that code is a LIE for non24! I naturally sleep through a lot of the daytime!

It is absolutely hilarious that I started up the "wakefulness-promoting" NDRI again (Sunosi) and once again it has effortlessly and completely cured my intractable lifelong non24 that, unmedicated, regularly results in me sobbing "but why can't I SLEEP" at hour 56 of being awake

If I could wave a magic wand and cure one of my disorders, it wouldn't be the non24, it would be the MCAS (because I strongly suspect curing that would also cure the non24). But it's definitely #2.

But if you too have terrible horrible non24 -- like seriously, my natural "day" is a freerunning variable period between 30 and 70 hours and I've had to arrange literally my entire life around accommodating it -- the drug I'm taking is Sunosi (solriamfetol).

Because I like my life now, quite a lot actually, but if I'd gotten on the Ritalin as a teenager and it both treated the ADHD and mostly fixed the non24, dear *God* would I have had so many more options and less trauma.

(For the record: two painkillers, one long-acting, one PRN for short term breakthrough; one ADHD med; one PRN benzo for muscle relaxant/pain/MCAS; one NDRI for my non24 + because for SOME fucking unknown reason it significantly helps my POTS.) (PRN = "pro re nata", aka "as needed".)

But it can happen in sighted people too and we have no fucking clue why our chronobiology is always flashing 12:00 like after a power outage. (There's a theory that sighted non24 is actually a completely different disorder than no-light-perception non24 because of it.)

Hilariously, melatonin makes me horribly wired and twitchy! I have finally dialed into what's causing the non24, which has baffled doctors for *decades*: it's a combination of too much histamine (like everything else in my freaking life lol) and not enough norepinephrine and dopamine

Normal person with insomnia: still has entrainment, it's just really prone to slipping away from "real" timing. Person with non24: no entrainment, like, at all. Since light is one of the most powerful entrainment cues, it's why non24 is most common in people with no light perception.

I first noticed the "a NDRI cures my non24" when I was on Ritalin for the ADHD but the timing was finicky as fuck, if I missed the afternoon dose by even 10 minutes, I'd slip and have to slide around the clock again, and I'd still have a few insomnia episodes a month.
