Background/aims Primary open-angle glaucoma (POAG) and primary angle-closure glaucoma (PACG) are distinct diseases, yet many glaucoma cases in population-based datasets lack subtype specification. We assessed incidence patterns of glaucoma subtypes in China and the UK and used genetic evidence to infer the likely subtype composition of cases recorded as unspecified glaucoma.Methods Incident primary glaucoma was identified from linked inpatient records in the prospective China Kadoorie Biobank (CKB; n=512 504) and UK Biobank (UKB; n=492 329) studies. Cohort-specific phenotyping algorithms defined POAG, PACG and unspecified glaucoma. Adjusted incidence rates were estimated by direct standardisation. To support subtype inference, polygenic risk scores (PRSs) were constructed using ancestry-specific genome-wide association studies, including a new East Asian PACG meta-analysis, and tested for association with glaucoma phenotypes using multivariable logistic regression.Results Over 12 years of follow-up, 1658 primary glaucoma cases were identified in CKB and 7643 in UKB. Most (>68%) cases lacked subtype specification. Incidence increased with age and was twofold higher among women for PACG in both cohorts and for unspecified glaucoma in CKB. In UKB, POAG incidence was fivefold higher among Black than White participants, with a similar but attenuated pattern for unspecified glaucoma. PRS analyses indicated that unspecified glaucoma closely aligned with PACG in CKB but was more heterogeneous in UKB.Conclusion Healthcare-recorded incidence patterns for POAG and PACG were consistent with established demographic risk factors, whereas unspecified glaucoma showed differences in subtype composition between populations. Integrating epidemiological and genetic evidence improves interpretation of glaucoma phenotypes when detailed clinical information is unavailable.

Primary angle-closure disease (PACD) remains a significant cause of visual morbidity globally, particularly in Asia, where over 18.5 million will have PAC glaucoma (PACG) by 2050. Although glaucomatous optic neuropathy is the most widely recognized cause of visual loss, PACD significantly impacts a range of anterior and posterior segment structures and physiological processes, such as corneal endothelial cell loss,1 trabecular meshwork structural changes and functional derangement,2,3 lens opacities, iris ischemia causing a dilated pupil and consequent degradation in vision, retinal vein occlusions,4 rapidly evolving pressure-related retinal ischemia,5 and increased surgical morbidity6 including aqueous misdirection and zonulopathy.

In this observational case-series study, we aimed to report malignant glaucoma (MGL) occurence in 4 consecutive cases (4 women; mean age, 60±11.0 y) after gonioscopy-assisted transluminal trabeculotomy (GATT) between January 2021 and September 2024 and to evaluate clinical characteristics and treatment outcomes. All patients had primary angle closure glaucoma (PACG) before GATT. They were hypermetropic (+1.75 to +2.00 diopters) and had 360-degree peripheral anterior synechia (PAS) on gonioscopy. Axial length and anterior chamber depth values ranged from 20.85 to 21.22 mm and 1.92 to 2.04 mm, respectively. Preoperative plateau iris appearance was confirmed by using ultrasound biomicroscopy and anterior segment optical coherence tomography in all cases. Intraocular pressure (IOP) ranged from 36 to 45 mm Hg before GATT with maximal antiglaucoma therapy. MGL was diagnosed on the first postoperative day after GATT. IOP levels at the time of MGL diagnosis ranged from 19 to 29 mm Hg. Due to no response to initial medical treatment, one case underwent pars plana vitrectomy with subsequent Nd:YAG laser hyaloidotomy for MGL recurrence, and, other 3 cases received irido-zonulo-hyaloido-vitrectomy (IZHV). The incidence of post-GATT MGL development was 1.3% (4/300 eyes). We first describe post-GATT MGL occurrence in 4 consecutive cases that had advanced PACG. They all had preoperative plateau iris appearance. Proper detection and adequate treatment of MGL are necessary to provide good IOP control and to prevent further angle damage after GATT.
