Adding a PENFAST dot phrase like a boss

I actually had a conversation with a resident while I was on my hospital rotation like "yeah his PENFAST score ~is 1, but just to be clear if he had anaphylaxis to penicillin in front of you this morning I would STILL tell you to give ceftriaxone over levofloxacin"

Talk to your patients about their antibiotic allergies- inappropriate allergy labeling has direct consequences:
1) not all drug reactions are allergies
2) reactions can go away over time
3) use tools like PenFAST to assess

The components are the same as PENFAST.

Score <3: 5% risk of allergy
Score 3+: 20% risk.

Haven’t seen much more on it since this paper, but you want a scoring system? There you have one!

not even kidding I have a long-term goal of getting a program off the ground where pharmacists can do direct delabeling and amoxicillin oral challenges for patients with PENFAST scores of 0-1, Alberta is the perfect place for it
