Though the larger reason for this is so that if either is captured in a time of war they’re protected by the Geneva Conventions:

I don’t doubt that the PHS and commissioned corps could be reformed, but the PHSCC should have an politically-appointed, civilian leader for the same reason that all the other uniformed services do: civil-control ensures the corps serves the public rather than self interest.
