Stop doing PSA tests to "diagnose" Prostate Cancer.
It's a rubbish non-specific test which identifies infection, BPH, and mostly very low-risk indolent Gleason 6 cancers, that will likely never need treating.
Always use MRI set to detect PIRADS 4 & 5 instead. #medsky

Stop doing PSA tests to "diagnose" Prostate Cancer.

I, a patient, have no idea! My Dr tried to convey that it just means we need to do a biopsy. Which is fine. Then I saw the description of what PIRADS “means” and swung to “like 80% chance” which isn’t right at all. I don’t know if I had a Grade prediction but I wouldn’t have gotten 37%.

Since then, I have had two more MRI scans. One came back with a PIRADS score of 3, & the most recent, done this week, has come back with PIRADS = 2 (clinically significant cancer is unlikely to be present). I think it highly unlikely that my urologist would therefore recommend a further biopsy.

I had an MRI scan of the prostate about six years ago and it came back with a PIRADS score of 4 (PIRADS 4: clinically significant cancer is likely to be present). This was followed by a biopsy in which found 2 samples out 20 were found to contain adenocarcinoma cells.

Whilst rated at PIRADS level 2, there is NO emergence of suspicious cells in Prostate.
Basically, I have an inflamed Prostate (prostatitis) on a mild to moderate rating.
IF annual PSA ratio tests worsen then I go to Specialist.

Looking for the best papers on the positive and negative likelihood ratio for prostate MRI. What is LR+ and LR- separately for PIRADS 3, 4 and 5? Problem is, this can only be derived from a study where everyone had a biopsy or long-term follow-up.

It would normally follow two elevated PSA tests. 10 years ago they would do a biopsy. Now they would do Multiparametric MRI first, which can be set to ignore very low-risk cancer foci.
PIRADS 4 indicates biopsy. 3 would now be surveillance even if G6 suspected.
