The NHS has been doing this for decades. They claim intake growth is a surprise so it's never funded. This causes backlog. Gatekeeping is an asscovering powertrip: unnecessary and harmful. imo all the new arrangement does is throw a veneer of respectability over the top to mask a broken system.
Gender services in the UK are designed with a bottleneck for first appointments.
Once in, they’re designed to trap trans people in a perpetual cycle of proving themselves.

It's clear from the number of total appointments that services remain focusing on people 'already in the system', and other data from England shows that people are remaining in the system for longer.
