Most (all?) MTAs still fall back to plaintext because some god-forsaken MX of some obscure domain still chokes on STARTTLS and it'll probably never get updated.

Sort of fixed nowadays, the entire in-transit session is usually encrypted (STARTTLS or SMTPS).
S/MIME or PGP over cleartext SMTP has (had?) this problem, but negligible usage anyway.

FYI: Using @baileytownsend.dev's gatekeeper my PDS now support 2FA.
It was a dumb configuration issue combined with a very strict MUA, that refused to upgrade to STARTTLS on its own.

"STARTTLS stripping":
OfflineIMAP before 8.0.3 trusts the server with their STARTTLS
capability prior to authentication, which allows STRIPTLS/man-in-the-
middle attacks, taking over the connection and extracting account
credentials in cleartext.
