It’s also hard to tell if worse or new kinds of confusion (hearing/seeing things, thinking someone is entering their home) is due to a *new* medical issue (infection, stroke, etc) or is expected worsening of chronic issues.

Reasonably certain this is Evekeo: an immediate- release amphetamine stimulant used to treat narcolepsy and ADHD. The only other pills that size, shape, and color offered in the US are Synthroid (he wouldn't be taking that) and Tylenol with Codeine (slow- release, no reason to hold it in your mouth)
