Du 5 au 12 octobre, des équipes MSF ont pu se rendre à Tougan pour y évaluer les besoins médicaux des habitants, résidents comme déplacés. La mise en place de nouveaux vols intérieurs par le service aérien d'aide humanitaire des Nations unies (UNHAS) a notamment permis cette évaluation sanitaire huit mois après que deux employés de MSF ont été tués dans l'attaque de leur véhicule à proximité de Tougan. *« Après la mise à l'arrêt de nos activités sur place, après des mois de privations et de violences endurées par la population de Tougan, nous nous attendions à faire face à une situation humanitaire très difficile mais le constat, sans appel, de la détérioration nutritionnelle des enfants a confirmé nos craintes. Les résultats du dépistage nutritionnel effectué auprès de 882 enfants de moins de cinq ans indiquent que 19,7% étaient atteints de malnutrition aigüe tandis que 9,6% souffraient de la forme la plus sévère de la malnutrition aigüe », explique Moctar Abass, responsable adjoint des opérations MSF au Burkina Faso . *Ces résultats, obtenus en faisant du porte-à-porte dans les quartiers qui concentrent le plus de personnes déplacées, témoignent d'une situation de crise nutritionnelle très largement au-dessus des seuils d'alerte mondiaux. En partenariat avec les autorités sanitaires burkinabè, MSF se prépare à renforcer la prise en charge des enfants malnutris à Tougan en soutenant les activités des centres de santé et des agents communautaires et en développant des capacités d’hospitalisation pour les enfants les plus sévèrement touchés. Des médicaments et des stocks d’aliments thérapeutiques prêts à l’emploi ont été acheminés par voie aérienne pour aider le personnel du ministère de la santé encore sur place. Tougan abrite environ 36 000 personnes déplacées, dans un pays qui en compte aujourd’hui plus de deux millions selon les chiffres de la commission nationale en charge de les enregistrer. « La situation à Tougan, où les gens nous disent ne pas pouvoir cultiver leurs champs en dehors de la ville et dépendre quasi exclusivement des convois militaires pour leur approvisionnement en vivres, n’est malheureusement pas isolée. Il y a de fortes raisons de penser que d’autres localités actuellement sous blocus subissent un niveau de crise nutritionnelle similaire », rappelle Moctar Abass. Depuis le début de l’année 2023, la population de la Boucle du Mouhoun est prise en étau par les affrontements, attaques et représailles qui se sont intensifiés dans cette région jusque-là moins touchée que les régions de l’Est, Centre-Nord et du Sahel par le conflit opposant les forces de sécurité du pays à des groupes armés. A travers le pays, le modus operandi de ces derniers pour enclaver des dizaines de localités se répète avec la même brutalité : les habitants des villages voisins sont sommés de quitter les lieux, attaqués et parfois massacrés, des mines et autres engins explosifs sont disséminés, les mouvements d’entrée ou de sortie de la ville visée sont interdits et des contrôles effectués dans les transports pour s’en prendre aux habitant qui oseraient défier l’interdiction. Rapidement, les vivres, médicaments et produits de première nécessité viennent à manquer, les prix flambent, et les ravitaillements dépendent principalement de convois militaires et de l’aide humanitaire.

Almost 20% of the children screened by Médecins Sans Frontières in this town in northwest Burkina Faso were found to be suffering from acute malnutrition. Between 5 and 12 October, MSF teams were able to go to Tougan to assess the medical needs of its inhabitants -- local people and the displaced alike. The new domestic humanitarian flights set up by UNHAS (United Nations Humanitarian Air Service) facilitated the medical assessment conducted eight months after two MSF employees were killed in an attack on their vehicle near Tougan. "Given that we had suspended activities in the area and the months of deprivation and violence suffered by Tougan's inhabitants, we were expecting to find a very serious humanitarian situation. The results of the nutritional screening of 882 children under the age of five years showed that 19.7% were suffering from acute malnutrition and 9.6% from its most severe form. The evidence of the deterioration in the children's nutritional status confirmed our fears," explains Moctar Abass who is deputy head of MSF operations inBurkina Faso. Obtained by going door-to-door in neighbourhoods sheltering the most displaced people, the screening results point to a nutritional crisis well in excess of emergency thresholds. Working in partnership with Burkina Faso's health authorities, MSF is now preparing to ramp up the provision of treatment for malnourished children in Tougan. This will include supporting community health workers and activities in health centres and increasing the inpatient capacities to treat the sickest children. Drugs and stocks of ready-to-use therapeutic foods have also been flown in to assist Ministry of Health personnel who remain in the area. Around 36,000 displaced people are sheltering in Tougan in a country, which, according to the statistics provided by the national commission charged with registering refugees, is already hosting more than two million. "People in Tougan tell us that they're unable to cultivate their fields outside the town, they are almost entirely reliant on military convoys for food. Sadly, their situation is not unique.
