Left image has been claimed to be debris from this Falcon 9 upperstage re-entry fell in Poznan,Poland, currently unconfirmed.

Indeed it looks like a COPV, picture on the right shows 1 that was found in Washington state after another Falcon 9 re-entry

The real failures were that another upperstage engine failed and the booster wasn't able to soft land, two things very much necessary for SpaceX's plans. Not the controlled landing that went exactly as designed.

If you want a real answer its basically because Starship could've gone orbital on its 3rd flight, but because its aiming to have a reusable upper stage, suborbital trajectories are a safer to test it. New Glenn's dipsoable upperstage means they didn't have the same restrictions on testing.

I'm skeptical anything will be visible, especially since it's impacting the lunar dayside. Yes, it's a 3+ tonne upperstage, but the velocity is kinda "meh".

But, I think when the expendable Falcon9 upperstage burns in the atmosphere, it might cause 2 tons of aluminium oxide.

The sooner the starship starts to operate, the sooner aluminium oxide pollution can start to get lower.

With SpaceX, if they have an issue with an upperstage or data looks sketchy…they swap it out. That’s all they need to do

That’s why out of the 600+ operational missions they’ve conducted only 3 have failed. They only move fast and break things when no payload is on the line

Playing with rockets in Kerbal Space Program with the RP-1 modpack :3. This is both in its two side boosters and 4 side boosters configuration
A 162.976 Tonnes rocket, standing at 23.4 meters in Height, 3.1 meters in Width, and 8.9 meters in length. It has a two LR87-AJ-1 liquid boosters colored in white, red, and green in various stripes, strapped onto a dual LR105-NA-3 Core Stage colored in black and white. The upperstage is an XLR81-BA-5 (known as Agena-A). This stage is colored in cyan and a very light pink with the fuel tanks having the texture patterns of the Able upperstage used on the Thor-Able rocket, the XLR81-BA-5 is also covered by an interstage that is white and cyan. Atop the upperstage there is a completely Cyan fairing where payloads are put. The rocket is called bIQ'a' Sargh which is Klingon for badger, and it can lift 2.28 tonnes to Low Earth orbit A 241 Tonnes rocket standing at 23.7 meters in Height, 8.9 meters in Width, and 8.9 meters in length. It has 4 LR87-AJ-1 liquid boosters colored in white, red, and green in various stripes, strapped onto a dual LR105-NA-3 Core Stage colored in black and white. Here's the payload, though I think the solid rocket kickstage of the GCRC might not be necessary anymore, at least unless I do like a Trans-Lunar injection burn, but that's for a later program than the one I'm doing in RP-1. Anyway, it reached the orbit. Now I just need to do science experiments

Second time is the charm. I tweaked the settings of mechjeb pvg and removed the force from the decoupler. Also added coasting to apoapsis, which helps a lot and makes it so it does the launch in only two stages
The Earth is in the background. The Upperstage decouples from the orange-colored booster, The Agena is the engine used on the Upperstage and the fuel tank its attached to is colored green and cyan, the satellite's fairing still attached. It is coasting for around 226 seconds to reach near apoasis and then ignite its engines, which will save a ton of delta-v for the target orbit of the contract

Did I read correctly that IFT-9 failed in every aspect:

- booster lost connection (resulting to "hard splash")
- payload was not deployed
- upperstage lost control
