The only reason it keeps coming up is because you NEVER APOLOGIZE! Not when it happens in real time, not LATER when it is brought up to you, not when it is EXPLAINED IN DETAIL how it harmed people, NEVER.

So it keeps returning. Over and over again. Many things are like that when unaddressed.
