Ich fange mal an:
1. Bupi 0,5% hyperbar (Größe in m = ml ~ 1,70 m Patientin = 1,7ml Bupi) + 1 ml = 5mcg Sufenta in separater Spritze
2. Bupi 0,5% isobar pauschal 2ml + 1 ml Sufenta
Bin gespannt wie es bei euch so läuft!

acho que a bupi finalmente vai fazer algum efeito, porque volta e meia acendo um cigarro e acho asqueroso (mas ainda termino porque tá caro).

We use noradrenaline on a periferal iv, so start that at induction.
I’d combine regional & GA/sedation.
FIBlock + GA
or
miniSpinal = 4mg bupi + opioid (we use Sufentanil)
or spinal & catheter titrate tiny doses of LA.
avoid midazolam.

A 5ml STB with lidocaine will allow an MUA or a shoulder reduction or bupi for post op pain
A larger volume of bupi (for example) would allow more extensive shoulder surgery awake
