Attention all personnel.

After seven days of “research”, I can safely say Cefdinir takes line old Chinese food.

That is all.

Oh, and I no longer have an ear infection. HAZAH!

I'm so curious. I could readily believe cefdinir use is higher among MAGA doctors.

CEFDINIR ALSO BAD APPARENTLY

I have also still not heard from my doctor at all about the first 3 days of cefdinir making my belly feel like I just did 20 situps and if I should worry about that. I just want competent doctors who give a shit dude

📊7.3M kid encounters (2016-23); cefdinir in 10.4%, ↓ to 7.6% post-2020. Highest use in 1-5yrs & UTI <1yr (OR 9.38). NH Black & urgent care less likely Rx.🦠
Trends and demographic predictors of cefdinir use for common bacterial infections in children enrolled in Medicaid, 2016–2023
View abstract Objectives:To determine trends and predictors of cefdinir use for common pediatric infections in the United States.Design:Retrospective cohort study, 2016–2023.Setting:U.S. outpatient encounters (ambulatory, emergency department, urgent care) included in the Medicaid MarketScan database.Patients:Children aged 0–18 years diagnosed with bacterial rhinosinusitis, pharyngitis, urinary tract infection (UTI), acute otitis media (AOM), or community-acquired pneumonia (CAP).Methods:We assessed cefdinir prescription rates by diagnosis and year, comparing to other commonly administered antibiotics. We used logistic regression to identify demographic predictors of cefdinir use. Results:Among 7,346,547 encounters, most common diagnoses were AOM (36.9%) and bacterial pharyngitis (36.2%). Patients were predominantly non-Hispanic White (53.0%), 1–5 years-old (41.1%), and seen in ambulatory settings (75.3%). Cefdinir was prescribed in 10.4% of encounters, ranking among the top prescribed antibiotics. After an initial increase, use declined post-2020 to 7.6%. In children 1–18 years-old, younger age was uniformly associated with higher odds of cefdinir prescription across all diagnoses. This pattern was inconsistent among infants <1 years-old across diagnoses but persisted for UTI, with ORs of 9.38 (95% CI, 8.91–9.88) for <1 years-old and 5.91 (95% CI, 5.76–6.06) for 1–5 years-old versus 15–18 years-old patients. NH Black patients and those seen in urgent care or the emergency department were less likely to receive cefdinir. Conclusions:Despite a small decline in use since 2020, cefdinir use remains substantial, especially among young children with UTI. Continued efforts to reduce inappropriate use and promote antibiotic stewardship are warranted.

1st line tx for UTI in kids=cephalosporins.

Cefixime/cefdinir easier dosing schedule than cephalexin.

My ENT seems to prefer cefdinir and tbh so do I bc it's once a day vs 2 or 3 and it's not nearly as gross as augmentin which is the devil's medicine

🚨 PK matters! Cefdinir showed higher treatment failure vs cephalexin for uncomplicated UTI (doi.org/10.1093/ofid...). Remember to account for bioavailability & urinary penetration when selecting UTI therapy. #AntimicrobialStewardship

I hear that. Maybe I’ve just gotten lucky but I’ve never had cefdinir fail for UTI (mainly use it in older women). W/ my population I have enough patients with baseline adherence barriers that all else equal I find the theoretical risk outweighed by the benefit of them taking the correct dose. YMMV

I’ve been using BID cefdinir instead of cephalexin whenever possible for a while now for just this reason. More expensive for uninsured patients but I can still get it for them for $15 or less.

💊 First-line oral stepdown agents:
• Levofloxacin
• Ciprofloxacin
These have excellent oral bioavailability and a high likelihood to achieve PK/PD targets for Enterobacterales bacteremia.

3rd line: high dose amox(clav), cephalexin, NOT cefdinir❗

There was a total of 185 cefazolin-resistant Enterobacterales isolates. Among these isolates, 88 were cefazolin-resistant but ceftriaxone susceptible; susceptibilities to oral cephalosporins were as follows: cefuroxime (79, 90%), cefixime (81, 92%), cefpodoxime (62, 71%), and cefdinir (77, 88%).
