tl;dr - there was a typo in dsm 3 that meant PDD-nos could be incorrectly diagnosed between 1994-2000 (and probably longer), then pdd-nos got subsumed into autism. no one knows how many people this affected.

Third, there was an error in the final manuscript. It is not well known, even among experts, but in 1993, when the authors of the child psychiatry section of the DSM were editing the proofs of the new DSM-IV, which would be published in 1994, they missed a critical mis-take. For PDD-NOS, the largest group of autism spectrum disorders, the authors had intended to write as the criteria, "impairment in social interaction and in verbal or nonverbal communication skills." A different text was accidentally published. It said, "impairment of reciprocal social interaction or verbal and nonverbal communication skills, or when stereotyped behavior, interests, and activities are present." The authors had wanted someone to qualify as autistic only if they had impairment in more than one area, but the criteria, as published, required impairment in only one area for a diagnosis of PDD-NOS. The threshold for PDD-NOS was thus lowered considerably, without anyone realizing it until it was too late. In a small study, the results of which were reported in a letter to the editor of the Journal of the American Academy of Child and Adolescent Psychiatry, the authors of the DSM-IV PDD criteria found that in 75 percent of cases in which a clinician ruled out PDD-NOS, the DSM incorrectly identified the children as PDD-NOS. The editorial error, made in 1994, was cor-rected, of course, but not until a revised edition of the DSM-IV was Published in 2000. By that time, psychiatrists, psychologists, and epidemiologists throughout the world had been following these extremely broad criteria for six years, during the period when the so-called "epi-demic" emerged. Although it is unknown whether those diagnostic criteria produced a large number of false positives, they certainly didn't reduce the number of cases. It is also unknown whether the corrections made in 2000 made a difference in the clinician's office, since by that time the 1994 criteria had already become a part of everyday diagnostic practice. The foundation for twenty-first-century diagnostic ambiguity was now firmly set. Today, a child with autism is a child "on the spectrum," a classification that tells us just how murky the diagnostic waters of autism have become. It is so imprecise that it tells us mainly that the person has deficits in communication and social interaction, has restricted interests and activities, and falls somewhere between profoundly mentally retarded and exceptionally intelligent. And now, PDD-NOS, at least in common usage, is subsumed under the category of "autism spectrum disorders." The rates grew as the concept of autism changed. Of eight studies conducted between 2000 and 2003, six found rates of between 52 per 10,000 and 67.5 per 10,000 (that is, as high as 1 in 155).
