I am personally annoyed by the fact that hypocalcemia causes tetany and hypercalcemia causes weakness and not the reverse as the electrochemical gradients would imply. Stupid sodium channels

I sometime attend in an LTAC and I regularly see hypercalcemia. When I work it up, by far the most common diagnosis is immobilization hypercalcemia (low PTH without PTHrp, 1,25 OHD, a negative SPEP/UPEP, negative skeletal survey)

I can't find any references to support this observation

It can also cause agitation, cause some people to have suicidal thoughts or become more depressed. It also may cause gynecomastia and hypercalcemia, and increase cholesterol.

It’s also habit-forming.

It can also decrease the amount of sperm men make and affect their ability to have children.

When evaluating hypercalcemia, initial workup includes parathyroid hormone (PTH) and Vitamin D-25 levels. When those come back normal or inconclusive, you then check a Vitamin D-1,25 level, which will come back inconclusive. Then you check a PTH-rp level, which will not come back in your lifetime.

15K of vitamin D for two months would likely cause at least a couple cases of hypercalcemia.

Caitlin update. She remains in critical care in the Lurie Children's Hospital PICU. She is rewatching old Supernatural episodes. Otherwise, not a great day. Her hypercalcemia is worse & remains a mystery. There are many other not great things going on. Good thoughts are needed.

Caitlin update. She remains in critical condition in the PICU. Though her urosepsis is improving, her hypercalcemia is a little worse. Multiple other problems today, some new. Caitlin remains aware, and is in good spirits. She is watching hospital TV.

Caitlin update. It has been a ride, but all is back to being calm. Caitlin still needs a bipap with oxygen at all times. Taking her off it went... poorly. They are battling hypercalcemia and urosepsis. Probably there are additional issues. We will be in PICU for awhile.

Caitlin update. Not great. Lots of nasty side effects to the zoledronic acid. It barely made a dent in her hypercalcemia. She is exhausted and not opening her eyes. More fluid overloaded. Lots of new electrolyte derangements. Numerous tests & treatments today. 😔

Geezus aich christos, Vit A is toxic and can cause severe hypercalcemia, children are much more susceptible. Do not take supplements!

Caitlin update. The hypercalcemia won’t budge, so last night she received another infusion of zoledronic acid. Calcium is *still* up, and now she is in pain. No desats in a bit, but her weight increased again. Things are very much not great. 😔

Parathyroid adenoma —> hypercalcemia —> neuron transmitter disruption —> fatigue, anxiety, depression, cognitive disfunction, and even psychosis, for all the doctors in the house.

So if you have any of these mental health challenges maybe get a blood test. Might change your life. For good!

Rare but can be devastating if it happens and it doesn’t really have a treatment beyond supportive care and managing the individual abnormalities (i.e., hypercalcemia) that arise. Children are at higher risk of developing it than adults.

Grady update:

Good news is his vet has consulted some internists and oncologists and given all they've seen, it's not lymphoma.

Bad news is that leaves idiopathic hypercalcemia. That means his calcium is up and there's no discernible cause.
