CN XII (Hypoglossal Nerve) 👅
Tongue deviation from stroke or trauma. Rare standalone, but a key lower motor neuron sign.

Stick it out there!

Hypoglossal nerve stimulator, on lateral x-ray. For treatment of obstructive sleep apnea resistant to other measures. Electrodes (3 dots) are placed at the hypoglossal nerve, & will fire to contract muscles of the tongue, when it detects that you have stopped breathing, thereby opening your airway.

New paper out today led by @gallinaciega.bsky.social

We show that seed-eating songbirds have enlarged a somatosensory nucleus, especially the part associated with the tongue, suggesting that seed-cracking is dependent on a fine sense of touch.

Hypoglossal nerve stimulation for obstructive sleep apnea shows downstream health benefits
Researchers estimate that more than 80 million U.S. adults are living with obstructive sleep apnea, a condition in which the soft tissue at the back of the throat collapses and blocks the airway during sleep.

Implanted nerve stimulation for obstructive sleep apnea found to be safe and effective
A randomized controlled trial evaluating the safety and efficacy of proximal hypoglossal nerve stimulation, or pHGNS, (an implanted stimulation therapy targeting specific sectors of nerves in the tongue) for obstructive sleep apnea (OSA) found that pHGNS significantly reduced breathing interruptions during sleep and improved patient-reported daytime sleepiness without any significant adverse events.

Of course!! I’d been doing research for a few years on this, Genio his both sides of the hypoglossal nerve rather than just one side like Inspire, and doesn’t require an implanted battery. Here’s a surgeon from the NHS’ video about it that’s really informative
