I'm thinking "multi-infarct dementia" is a co-morbidity, too. High risk for this with A-fib. It would explain the rapid progression of his cognitive loss.

Likely multi-infarct dementia (subclinical small strokes repeatedly over years leading to dementia) and if he's got liver issues, hepatic encephalopathy will worsen his cognition more.

Later--Labs pretty normal. Imaging showed a splenic wedge infarct. Atraumatic splenic infarct without a good etiology is what I ended up with as dx. Attending made sure to let me know he'd be talking about the case with resident...
