I’ve often said that keratoderma blennorrhagica is clinically indistinguishable from pustular psoriasis, at least to the untrained eye.

You're close, very close! Punctate palmoplantar keratoderma, type 1. Clinical v important. Keratotic papules can drop out leaving pits (this was reported as "cyst") but we can see the hyperkeratosis here too. Clinicians wanted histo review.
