If your ED doesn’t carry buprenorphine (assuming a solidly high COWS), at least treat the symptoms. Zofran (or escalate), lomotil, fluids. Consider a benzo if anxious.
A patient in abject w/d misery:
a). Will continue to deteriorate medically, and
b). Is at very high risk for overdose.

DEET, although ticks aren’t as bad as VT
Bear spray & a bear” banger” used it when I lived in BC, sounds like gunshot. Air horn works good too( used that in Tanzania)
SOCKS! Lots o socks. ☺️
