an illustration of the lithotomy procedure with a latera cross section of the pelvis to show the internal anatomy. the patient is supine, a large bladder stone in the urinary bladder. a probe is inserted through the penis and held with an ungloved hand. two other hands press down on the patient's pubis and retract the testicles. an incision is made just posterior to the testicles with a long scalpel which inserts through the prostate into the bladder, just touching the stone.

traumatic injury to the pelvis can potentially internally divide the urethra just distal to the prostate. because the urethra contracts, re-connection may require shortening the distance by chipping the pelvis and passing the urethra around other structures
