4/CMIs inhibit actin-myosin interaction, decreasing cardiac contractility, reducing LVOTO, and improving cardiac filling pressure. CMIs can be beneficial for patients with oHCM who do not experience adequate symptomatic relief from first-line drug therapy (eg, beta blockers).
It was found to be ineffective only in NON-obstructed HCM. If you have HCOM (or oHCM), which is HCM with obstruction, it most definitely works to relieve the obstruction, which is the primary source of all the symptoms like shortness of breath.
