One of my sickest patients with ME was labelled with psychogenic polydipsia and denied IV saline. Patrick Ussher explains ‘this myth of psychogenic polydipsia’ in this excellent podcast by my colleague Julie Taylor. He also has a free downloadable book (see below).

The history of where the psychogenic basis comes from is very interesting - now taught as ‘fact’ in medicine.

This might be my nurse brain overthinking but I'm worried new cat might have diabetes. Possible polydipsia, difficult to measure polyuria, but even though she has the same litter as Dottie, her litter smells sweet, I think. Maybe? Time for vet, obviously.

💡 Does arginine stimulate vasopressin via urea?

✅ Urea increased after arginine.
✅ Copeptin increased in healthy adults & primary polydipsia, but not in AVP deficiency.
❌ Urea & copeptin correlation? Weak.

Glucose IS an effective osmole in the absence of insulin or insulin resistance. So hyperglycemia does lead to polydipsia (and polyuria due to osmotic diuresis)

I had a younger patient that had polydipsia an if you didn't empty his urinal fast enough he'd drink it if he didn't have access to water

Many with excessive thirst get the "psychogenic polydipsia" label slapped on them. Yet experiments in the early 1800s showed excessive thirst can be relieved with IV water or saline – seems medicine has regressed!

Mom knows the signs and I have absolutely none. Vet taught her this acronym today:

PUPDPPP, representing Polyuria (excessive urination), Polydipsia (excessive drinking), Polyphagia (excessive appetite), and Panting.

I'm the dude whose HbA1c freaks out general practitioners but whose OGTT results (and overall presentation like, e.g., absence of polydipsia and polyuria) very firmly say, "Not diabetic."
