Un equipo de MIT y hospitales colaboradores presentó xvr, una IA que adapta en unos cinco minutos un modelo a la anatomía de cada paciente para alinear rayos X tomados durante una intervención con una tomografía o resonancia previa. El trabajo, publicado el 16 de septiembre en _Nature_ , busca convertir una imagen plana en una referencia tridimensional para navegar instrumentos. Sus resultados miden el registro de imágenes, no una reducción demostrada de complicaciones ni mejores desenlaces clínicos. ## Cómo xvr cruza los rayos X con el escaneo previo En intervenciones guiadas por fluoroscopia, como algunas angioplastias, los equipos médicos usan rayos X en tiempo real para seguir catéteres y otros instrumentos. El problema es que la imagen es bidimensional y no muestra por sí sola la profundidad ni la relación exacta del dispositivo con las estructuras anatómicas. El proceso de empatar esa radiografía con una tomografía computarizada o una resonancia magnética previa se conoce como registro 2D/3D. Hoy puede requerir ajustes manuales o sistemas entrenados para una zona específica del cuerpo. xvr intenta automatizarlo creando, a partir del escaneo de la propia persona, miles de radiografías sintéticas modeladas con la física de los rayos X. Con esos datos, el sistema ajusta un modelo previamente entrenado con más de 2,000 estudios tridimensionales de cuerpo completo. Según MIT, esa adaptación tarda alrededor de cinco minutos y el registro posterior se ejecuta en segundos. El propósito es aportar a los especialistas una referencia anatómica más clara mientras realizan el procedimiento, no sustituir su decisión clínica. ## Qué probaron y qué todavía falta El artículo reporta que xvr superó a otros métodos en exactitud de registro en datos de fluoroscopia reales. En uno de los conjuntos clínicos, con 50 angiografías por tomografía computarizada y 122 angiografías de sustracción digital de pacientes de neurocirugía, el error mediano de registro fue de 1.2 milímetros. Esa cifra describe qué tan bien se alinearon las imágenes, no la colocación final de un instrumento ni la tasa de complicaciones. Los propios autores piden cautela: la evaluación incluyó 21 sujetos de conjuntos públicos y 54 de fuentes clínicas privadas. Señalan que harán falta cohortes más amplias, más regiones anatómicas y estudios piloto que integren el sistema de forma prospectiva durante intervenciones. Además, la corrección iterativa que alcanza la precisión final añade entre uno y ocho segundos, por lo que todavía no equivale a una guía autónoma en tiempo real. La investigación abre una vía para que la navegación con rayos X aproveche mejor los estudios 3D que ya existen antes de una operación.
