I didn't love the split down the middle or the text round the edges, so I've covered it up as well as I can. It's not perfect but it'll do.

I am officially out of patience for 'main character has memory loss' as a plot device. It's both lazy and deeply offensive. Memory loss is a crippling, dehumanising medical condition, not a writer's tool.

Though you could use it for any TTRPG worldbuilding, I suppose. Just replace the rule for if you roll a 1 - maybe something like "Roll Twice and Combine Biomes"? Depending on how whimsical you're going with your environments.
