Not a perfect comparison but interesting nonetheless. Augmented UOP with the intervention vs placebo in CLOROTIC, EMPAG-HF and ADVOR

HCTZ: 26%
Empagliflozin: 25%
Acetazolamide: 12%

1) acetazolamide for iatrogenic contraction alkalosis is standard tx IMHO (most drugs are "off label") along with K repletion etc

2) PO acetazolamide probably fine, great bioavailability

3) prevention with amiloride is great, I love amiloride

4) triamterene is maybe nephrotoxic so less of a fan

People in the USA only know brand names for medicines and it’s wild. They never take acetaminophen, only Tylenol. It’s never acetazolamide, it’s Diamox. It’s wild.

lead to daytime hypoventilation and hypoxemia. Thoughtful use of acetazolamide in this scenario may help by lowering bicarbonate and restoring respiratory drive

It depends, what’s the cause and volume status? If there’s hypovolemia, give NS, if hypervolemic, acetazolamide works well.

"Retro" is usually reserved for fashion and music. Typically not something you see in medicine - unless it's acetazolamide I guess.

Got a patient from the US on chronic torsemide 100 mg OD and tolvaptan 15 mg OD and empagliflozin use with mild hyponatremia and hypochloremia and still gaining weight.

Rather than an algorithmic approach, try to tailor your adjunctive agents to the patient's biochemical milieu.

The patient w rising bicarb who needs ongoing diuresis? Great candidate for acetazolamide.

The patient w recurrent hypoK? Try adding a potassium-sparing diuretic (ie spironolactone).

Carbonic anhydrase activity contributes to lithium-NDI development, and acetazolamide attenuates lithium-NDI development in mice similar to thiazide/ amiloride but with fewer adverse effects 🐁
