Any experience with generic budesonide versus Tarpeyo(targeted release budesonide) for IgA nephropathy? All I can find are proteinuria studies and increased risk of systemic side effects. Blue Cross wants patient to fail generic budesonide for 3 months before approving Tarpeyo. #askrenal

I have a 72 year old on Automated PD since 2022 who initially defaulted PD for months and once recommenced on PD developed vomiting while on PD. Non compliant to follow-up and his last PET test in early 2023 showed a HA transporter.

Quick question - if I want to give Cefoperazone-Sulbactam for IP antibiotics for Automated PD with 10L exchanges can I use the Cefoperazone data in ISPD guidelines ie 500mg/L loading with 125mg/L for maintainence or is that overkill? #AskRenal

SONAR did in fact show that it was effective, but the premature termination and upstaging by CREDENCE sort of left atrasentan for dead…

Has any hospital instituted a reporting requirement that the eGFR cannot be reported if there is a rapid change in creatinine over a short period of time? Feels like it would reduce errors in drug dosing and aid with decision-making

Requiring a test 2-3 weeks after starting an SGLT2i and then dealing with the (expected yet surprising!!) eGFR drop
-> underprescription and unnecessary discontinuation.

Checking should be limited (high risk eg vol issues or if symptoms).

#askrenal @nephjc.bsky.social have a young male with multisystem sarcoid. Eyes, liver, hilar LNs and interstitial nephritis. Granulomas so big you could pick them out without magnification on histo slide! Any experience of anti TNF as steroid sparing for renal involvement?

In a patient suspected to have PA, with high aldosterone, could discrepancy between direct renin measurement (increased) and PRA (lower norm) be caused by a beta-blocker? Re RAS inhibition, no direct renin inhibitor, only ARB in the medication. Thank you! #askrenal
