Clem Burke bnwas my first celebrity sighting in NYC, on a trip during high school in the 1980s.
Whilst there may be another explanation it is hard to see any other reason than Watchkeeper incompetence/fatigue in this issue. The only other plausible scenario I can think of is the Watchkeeper collapsed and not enough time elapsed between that and the BNWAS activating before the collision.

Indeed. I'm not going to comment too much because I'm in the UK and theoretically contempt of court applies, but yes, BNWAS being off is certainly suggestive.

Top marks, BNWAS motion sensors continually reset by balloons motion to save the watchkeeper from having to move past the sensor ca. every 12 minutes. Less irritating for the watch keeper but what happens if he has a heart attack or stroke, or even just gets absorbed in something else ?

Just need some ornament hanging in front of movement sensor or a ball-bearing ball on radar/ecdis trackpad (or equivalent) to defeat the dreaded #BNWAS

BNWAS can be circumvented if not correctly set up (master key left available or password widely known)
I was once asked by a pilot if there was some special maritime event being celebrated because so many of the ships he visited had bunches of party balloons on the bridge.
Can u figure it out ?

Many ships have a BNWAS (button you have to press regularly to show you’re awake) on the bridge. I think trains have something similar too. Actually kind of surprising that airliners don’t, because this is a known problem.

2/x It also lists several shortcomings, such as a non-functioning bridge watch alarm. The Bridge Navigational Watch Alarm System (BNWAS) was disabled both before and during the grounding and must be repaired before the ship can sail.

I was fortunate in my career never to be in that position but it was always a worry when caught short during the daylight watches. I'd need to check the Regs now, but I recall our BNWAS was set to pre alarm after about 20mins so lots of time for things to go wrong.

Nobody keeping a proper watch and BNWAS switched off.
If it is switched off the OOW can be anywhere, not even on board! Look up Selandia Swan.

I've been on vessels with the password for the BNWAS activation/deactivation posted on a dymo label next to the control unit, so yeah, the fact that accidents like this is so (relatively) rare is a miracle.

Total redundancy in Radars and probably on GPS too.
Driver error in my opinion, nobody conscious on the bridge and the BNWAS switched off.

It's confusing. They would have no trouble spotting the tanker on radar. If asleep, or in capacitated km the bridge, the BNWAS should raise alarm to other personnel on the vessel.
